AED 2005 Test- This form can be filled out on-line and saved to your computer
- To print a copy, click on the print button on the form and mail or fax to SREMSC
- If you prefer, you can print a blank form, have the students fill it out, and mail or fax the completed form to SREMSC

The new“Cold Water Boating” DVD is now available. Dr. Gordon Giesbrecht, known as Dr. Popsicle for his work in cold water survival was a co-producer in this newest DVD developed by the Office of Boating Safety. Education Coordinator Joe McCullough, was the project manager, and can be contacted for presentations on cold water boating at 269-8704 in Anchorage.
Alaska residents can obtain copies of this DVD for no charge at Department of Natural Resources Public Information Centers (PIC) in Anchorage located at 550 W. 7th Avenue, Suite 1260, or the Fairbanks office located at 3700 Airport Way between the hours of 10:00am and 5:00pm.
Residents many also e-mail the Anchorage PIC office at dnr.pic@alaska.gov to request a copy be mailed for a small postage and handling fee. The PIC accepts Visa or MasterCard and will need a phone number to call you to collect this fee. Requests for mail orders can also be placed by calling the Anchorage PIC office at 269-8400 (dial 0 after 10:00am to reach PIC staff).
